HIS Registration Open.
The CMS Hospice Item Set (HIS) contractor has announced that the online registration applications for the CMSNet User ID and the QIES User ID are now available. Medicare-certified hospices will be required to complete and submit a HIS Admission record and a Hospice Item Set (HIS) Discharge record for each patient admission on or after July 1, 2014. These completed records are to be submitted to the Quality Improvement and Evaluation System (QIES) Assessment Submission and Processing (ASAP) system.
Hospice providers must register for two separate user IDs – one for CMSNet and one for QIES, which resides on CMSNet.
CMSNet User ID Registration
To request a CMSNet User ID, visit and follow the instructions.
Upon successful completion of the online registration, you will receive two emails. The first will contain the CMSNet User ID and the second will contain the password, installation links, FAQs and contact information.

QIES User ID Registration
The QIES User ID allows you access to the Hospice Submissions System for HIS Record submission to the QIES ASAP system and to the CASPER Reporting system for the Hospice Final Validation Report among other reports.
Once you have successfully logged into CMSNet using your newly created CMSNet User ID, you should access the “Hospice” link. This will provide access to the CMS QIES Systems for Providers-Hospice Welcome Page where you will find the “Hospice User Registration” link. This application is used to register for the QIES User ID.
