PDGM Handbook: Preparation from A to Z

The Patient-Driven Groupings Model (PDGM) is a new payment model for the Home Health Prospective Payment System (HH PPS) that relies more heavily on clinical characteristics and other patient data to classify home health periods of care into meaningful payment categories. It also eliminates the use of therapy service thresholds and separate payments for medical supplies. It’s scheduled to begin January 1, 2020 and is widely considered the most significant change to the home health industry since the implementation of the HH PPS in 2000.
